Position Title : Services Coordinator

Essential Functions:
  • Conduct initial assessments and periodic re-assessments to identify client needs and make recommendations regarding employment, education, and psychosocial services and referrals inside and outside of Laurel House.
  • Develop, implement and monitor individualized recovery plans with new and existing clients of Laurel House.
  • Participate in outbound opportunities (e.g., healthcare provider group meeting, NAMI conferences, presentations to high school counselors) to build referral network and increase visibility to Laurel House’s programs and services.  
  • Collect, update, maintain data for attendance, admissions, discharges for programs.
  • Conduct pre and post assessments for Thinking Well (cognitive remediation program) program participants, in advance of their enrollment in the program and at the completion of the program.
  • Facilitate or co-facilitate a weekly recovery workshop (e.g., Wellness, Coping Skills).
Scope

The Services Coordinator engages clients in every phase of the recovery planning process, from initial inbound referral, welcome and orientation to Laurel House programs, to completion of services and discharge.  The Services Coordinator also provides direct services to clients such as administering assessment tools and organizes and facilitates a weekly recovery workshop. 


Responsibilities
  • Create a welcoming environment for new program participants and their families; provide a comprehensive orientation to Laurel House services and activities.
  • Assess the needs and goals of new and returning program participants and work with them to develop individualized recovery plans.
  • Based on a needs assessment, make referrals of clients to programs and services both internal and external to Laurel House, including transition to case managers, employment specialists, education counselors and the Thinking Well program manager.
  • Process information and records for all new referrals and readmissions to Laurel House programs, including data entry of personal information of new and returning clients in DMHAS’s (Department of Mental Health and Addiction Services) DDaP system and other internal information systems used at Laurel House. 
  • Collect daily participant attendance data at the Resource Center, in addition to updating and maintaining admissions and discharge data for Psychosocial programs.
  • Organize the implementation of annual Consumer Satisfaction Survey across all Laurel House departments and ensure target participation rate is achieved.
  • Participate in pre-planned activities (e.g., social outings) with other Laurel House staff as needed and in periodic outreach to isolated or hospitalized participants.
  • Provide back-up coverage when it may be needed for Laurel House colleagues.
  • Facilitate or co-facilitate one weekly recovery workshop or group (e.g., Wellness, Coping Skills).
  • Participate in DMHAS, clinical and professional development trainings for staff as required.
  • Represent Laurel House at provider meetings in the community and at local community events and regional meetings promoting Laurel House services.
