Position Title : Client Intake/Provider Outreach Counselor
Department Unit : Social Rehabilitation
Reports to : Clinical Director, Resources to Recover

Essential Functions:
  • Conduct initial interviews at the Resource Center with prospective clients and/or their families to determine which programs will best assist the client; refer clients to the programs. Collect, update, and maintain data for program attendance, admissions, discharges.
  • Cultivate referrals to Laurel House programs (e.g., Thinking Well/Cognitive Remediation, Supported Education and Supported Employment) and increase visibility of services by meeting with healthcare providers, guidance counselors, and community agencies in the 8 towns served by Laurel House.
  • Facilitate or co-facilitate a series of workshops (e.g., Wellness, Communication Skills), along with other members of the Laurel House team.
Scope:

The Client Intake & Provider Outreach Counselor engages program participants from the initial inbound referral, welcome and orientation to Laurel House programs, to completion of services and discharge.  The Counselor also provides direct services to participants such as facilitating, along with other members of the Laurel House team, social rehabilitation skill building workshops. 


Position Responsibilities
  • Create a welcoming environment for new program participants and their families; provide a comprehensive orientation to Laurel House services and activities.
  • Assess the needs and goals of new and returning program participants.
  • Based on a needs assessment, make referrals of participants to programs and services both internal and external to Laurel House, including transition to case managers, Education Counselors, Employment Specialists, or the Thinking Well Coordinator.
  • Needs assessments may be conducted in the community (e.g., at healthcare centers, on school/college campuses, at libraries).
  • Process information and records for all new referrals and readmissions to Laurel House programs, including data entry for new and returning participants in DMHAS’s (Department of Mental Health and Addiction Services) DDaP system and other internal information systems used at Laurel House. 
  • Collect daily participant attendance data at the Resource Center, in addition to updating and maintaining admissions and discharge data for Social Rehabilitation programs.
  • Represent Laurel House at provider meetings in the community and at local community events and regional meetings promoting Laurel House services.
  • Facilitate or co-facilitate a series of workshops (e.g., Wellness, Communication Skills).
  • Assist the Clinical Director of Social Rehabilitation in implementing annual Consumer Satisfaction Survey for Laurel House and ensure target participation rate is achieved.
  • Participate in DMHAS, clinical and professional development trainings for staff as required.
