Position Title : Client Intake/Provider Outreach Counselor
Department Unit : Psychosocial Services
Reports to : Clinical Services Director

Essential Functions:
  • Build referrals to Laurel House programs (e.g., Thinking Well/Cognitive Remediation, Supported Employment, and Supported Education) and increase visibility of services by meeting with healthcare providers, guidance counselors, and community agencies in the 8 towns served by Laurel House. Cultivate healthcare practitioner and professional contacts who can make referrals to Laurel House programs.
  • Collect, update, and maintain data for attendance, admissions, discharges for programs.
  • Facilitate or co-facilitate a series of workshops (e.g., Wellness, Communication Skills), along with other members of the Laurel House team.
  • Conduct pre and post assessments for Thinking Well (Cognitive Remediation) program participants, in advance of their enrollment in the program and at the completion of the up to 30 session program. (Training will be provided on the use of the assessment tool.)

The Services Coordinator engages program participants from the initial inbound referral, welcome and orientation to Laurel House programs, to completion of services and discharge.  The Services Coordinator also provides direct services to participants such as administering assessment tools and facilitates a series of workshops. 

Position Responsibilities
  • Represent Laurel House at provider meetings in the community and at local community events and regional meetings promoting Laurel House services.
  • Create a welcoming environment for new program participants and their families; provide a comprehensive orientation to Laurel House services and activities.
  • Assess the needs and goals of new and returning program participants.
  • Based on a needs assessment, make referrals of participants to programs and services both internal and external to Laurel House, including transition to case managers, Employment Specialists, Education Counselors, or the Thinking Well Program Manager.
  • Needs assessments may be conducted in the community (e.g., at treatment centers, on school/college campuses, libraries).
  • Process information and records for all new referrals and readmissions to Laurel House programs, including data entry for new and returning participants in DMHAS’s (Department of Mental Health and Addiction Services) DDaP system and other internal information systems used at Laurel House.
  • Collect daily participant attendance data at the Resource Center, in addition to updating and maintaining admissions and discharge data for Psychosocial programs.
  • Implement annual Consumer Satisfaction Survey across all Laurel House departments and ensure target participation rate is achieved.
  • Facilitate or co-facilitate a series of workshops (e.g., Wellness, Communication Skills).
  • Participate in DMHAS, clinical and professional development trainings for staff as required.
