Position Title: Supportive Housing and Community Support Services Manager

Department Unit: Supportive Housing

Reports to: Vice President of Programs

Essential Functions:

  • Monitor the effectiveness and quality of Counselors’ support of program participants.
  • Report on the outcomes within various Department of Mental Health and Addiction Services (DMHAS) funded housing services programs (e.g., service plans for community clients and for Laurel House apartment building tenants).
  • Ensure that all programs comply with regulations and best practices.
  • Focus on independent living skills of program participants to promote their personal well-being, self-sufficiency and integration in the community.
  • Integrate case management support with services of other community-based organizations and agencies as needed. Maximize use of Laurel House’s own evidence-based programs, as appropriate for client strengths, such as Social Rehabilitation skill-building workshops, social events, Thinking Well (Cognitive Remediation), Supported Education or Supported Employment.
  • Provide case management to adults with mental health and substance use disorders. Provide direct service to up to 5 Supportive Housing program participants. Model behavior for 4 counselors (each of whom provides services for 15 clients) regarding meeting with your own clients in the community, documenting notes in a timely fashion, connecting clients with community resources.

Scope:

Ensure participants in program receive quality support from their Community Support Counselors with respect, empathy and consideration for the self-esteem of all participants. Provide skills training and support to promote independence and to improve quality of life. Facilitate individual and team meetings with staff. Attend meetings in the community to coordinate care for participants. Participate
as required in Stamford/Greenwich Housing First (SGHF) and Opening Doors Fairfield County (ODFC) initiatives and training (e.g., Housing Solutions meetings; Coordinated Access Network (CAN) meetings; once a year Point in Time (PIT) Count. Become proficient in reporting systems to DMHAS (e.g., DDaP) and CSH. Provide oversight and coaching to CSP Coordinator for clinical and professional development and to audit the Coordinator’s reporting of services and outcomes.

Position Responsibilities
  • Participate as an active member of the Program Leadership team to support the organization.
  • Provide supervision to case management staff in individual and team settings. Oversee the Counselors’ development of Recovery Plans and progress for their clients.
  • Ensure the team’s update and maintenance of all program related forms and client records in required systems (e.g., SalesForce, DMHAS DDaP and Targeted Case Management).
  • Work closely with Facilities Manager to coordinate the movement of individuals in and out of program apartments and to stay current on apartment conditions.
  • Collaborate with VP of Programs and Laurel House Clinical Director to assess/interview candidates for Laurel House apartments and other funded community apartment openings.
  • Serve as liaison between Laurel House Supportive Housing Program and required oversight by organizations; attend meetings, trainings, and provide periodic reporting. Laurel House, Inc. Supportive Housing Manager Internal Use Only
  • Develop relationships with community organizations and landlords as needed to promote Laurel House services and ensure good relations.
  • Provide case management for up to 5 individuals with mental health and substance use disorders. Develop an individualized Recovery Plan with each participant, directed at increasing skills in community activities and, longer term, integration into local communities.
  • For your client caseload and for the team’s clients, ensure the update and maintenance of participant records, including vital statistics with current contact information, benefits, income, treatment and medical providers, reassessments, progress notes, and all other records and reports required by program funding sources, in both electronic and paper versions as required.
  • Demonstrate knowledge of Supportive Housing client situations. Build your own familiarity and communication with them. Provide information to VP of Programs or as needed to CEO, for grant requests, outcome reporting and success stories.
  • Successful experience leading a team in daily aspects of providing case management.
  • Well organized and task oriented.
  • Able to work with community-based treatment programs to provide rehabilitative community support, crisis intervention, group and individual skill building in the community.
  • Ability to communicate clearly with staff and management and to engage with clients with mental health and substance use disorders and/or a history of homelessness.
  • Ability to advocate on behalf of program participants and to promote self-advocacy to exercise choice in pursuit of their recovery goals.
  • Teamwork with Laurel House managers as a hands-on leader for Counselors, making yourself available as needed for consultation or leadership in crisis situations.
  • Degree in social work, human services, psychology, preferred. Masters in social work a plus.
  • 2-4 years working in management in the social services field; mental health experience a plus.
  • Good oral and written communication skills.
  • Proficient in basic computer and telecommunication applications (e.g., email, text, internet, Word, Excel, data entry).
  • M-F schedule with one day per week on an 11:00 – 7:00 pm schedule and one day work from home (WFH). Set schedule for you and team to ensure coverage and allow a WFH for team.
  • Must possess a valid driver’s license and have and maintain a safe driving record.
