Position Title: Client Intake & Provider Outreach Counselor

Department: Social Rehabilitation Services

Reports to: Manager of Supported Education and Thinking Well

Essential Functions:

  • Conduct initial interviews in the community or at the Laurel House Resource Center with prospective clients and/or their families to determine which programs will best assist the client; refer clients to the programs. Collect, update, and maintain data for program attendance, admissions, discharges.
  • Cultivate referrals to Laurel House programs (e.g., Thinking Well/Cognitive Remediation, Supported Education and Supported Employment) and increase visibility of services by meeting with healthcare providers, guidance counselors, and community agencies in the towns served by Laurel House.
  • Facilitate or co-facilitate a series of workshops (e.g., Wellness), along with other members of the Laurel House team.

Scope:

The Client Intake & Provider Outreach Counselor engages program participants from the initial inbound referral, welcome and orientation to Laurel House programs, to completion of services and discharge. The Counselor conducts outreach to healthcare providers who make referrals to Laurel House programs. The Counselor also provides direct services to participants such as facilitating, along with other members of the Laurel House team, weekly or monthly social rehabilitation skill building workshops.

Position Responsibilities
  • Create a welcoming environment for new program participants and their families; provide a comprehensive orientation to Laurel House services and activities.

  • Assess the needs and goals of new and returning program participants.

  • Based on a needs assessment, make referrals of participants to programs and services both internal and external to Laurel House, including transition to case managers, Education Counselors, Employment Specialists, or to the Thinking Well program.

  • Needs assessments may be conducted in the community (e.g., at healthcare centers, on school or college campuses, at libraries).

  • Process information and records for all new referrals and readmissions to Laurel House programs, including data entry for new and returning participants in DMHAS’s (Department of Mental Health and Addiction Services) DDaP system and other internal information systems used at Laurel House.

  • Collect and maintain admissions and discharge data for Social Rehabilitation programs.

  • Represent Laurel House at presentations (e.g., healthcare fairs) in the community and at local community events (e.g., on college campuses) promoting Laurel House services.

  • Facilitate or co-facilitate a series of workshops (e.g., Wellness) and participate with other counselors in hosting a monthly Community Hour for clients.

  • Participate in DMHAS, clinical and professional development trainings for staff as required.

  • Good interpersonal skills and ability to engage and help motivate people to identify and achieve their goals.

  • Experience in Motivational Interviewing, a plus.

  • Knowledge of and respect for the life experiences, values, and multiple issues and challenges facing program participants.

  • Ability to communicate clearly and engage with program participants and other individuals with mental health disorders and sometimes a history of homelessness.

  • Ability to maintain appropriate professional boundaries and maintain strict confidentiality in accordance with HIPAA.

  • Good writing skills; experience in interviewing or conducting psychosocial assessments and documenting services and activities strongly preferred.

  • Computer literacy in use of MS Word, MS Publisher, MS PowerPoint, MS Excel, email, internet, and familiarity with standard database applications required.

  • Bachelor’s degree in Social Work, Mental Health Counseling or related Human Service field.

  • Experience working with people who have mental health disorders, preferred but not required.

  • Experience facilitating workshops or groups.

  • Good oral and written communication skills.

  • Must successfully pass the pre-employment drug test.

  • Must possess a valid driver’s license and a good driving record.

  • Must be vaccinated against COVID-19 to be able to work onsite
