Position Title : Supportive Housing Services Manager 
Department Unit : Supportive Housing 
Reports to : President and CEO  

Essential Functions:
  • Provide case management to adults with serious mental illness
  • Monitor the effectiveness of Community Support Service Programs
  • Report on the outcomes and challenges within various funded programs
  • Ensure that all programs comply with regulations and best practices
  • Enhance the independent living skills of program participants to promote their personal well-being, self-sufficiency and integration in the community
  • Integrate case management support with other community-based organizations and agencies, and evidence-based program supports provided by Laurel House Resource Center staff such as Supported Employment, Education, Psychosocial Rehabilitation, including Thinking Well/Cognitive Remediation


Ensure participants in program receive highest quality of support from their Community Support Counselors through empathy, respect and consideration for the dignity and self-esteem of all participants. Provide skills training and support to promote independence and improve quality of life. Facilitate individual and team meetings with staff. Attend meetings in the community to coordinate care for Laurel House Community Support Services participants and to participate as required in Stamford/Greenwich Housing First initiatives.

  • Provide supervision to case management staff in individual and team settings
  • Update and maintain all client records in required systems
  • Update and maintain all program related forms
  • Participate as an active member of the management team to support the overall organization goals
  • Provide information and reports to the Vocational and Community Support Services Director for grant requests and reports, newsletter, and success stories
  • Work with the Director of Operations to coordinate the movement of individuals in and out of program housing units
  • Develop relationships with community organizations to promote wrap-around services for Laurel House program participants
  • Serve as liaison as directed between Community Support Services Programs and other various community organizations through meetings, trainings, and other learning opportunities
  • Provide case management for up to 10 individuals with serious mental illness and some of those with co-occurring disorders such as substance use disorder
  • For own caseload develop an individualized Recovery Plan with each participant, directed at increasing participants’ skills in community activities and, longer term, full integration into their local communities
  • Oversee development of Recovery Plans for Community Support Counselors
  • Update and maintain records for participants, including vital statistics with current contact information, service plans, reassessments, progress notes, and all other records and reports required by program funding sources, in both paper and electronic versions
  • Provide support and continuity of service in Laurel House apartments and scattered site apartments (other landlord), Community Support Services office, and community.
