Position Title : Supportive Housing Counselor
Department Unit : Supportive Housing 
Reports to : Supportive Housing and Community Support Manager 

Essential Functions:
  • Provide case management to adults with serious mental illness.
  • Enhance the independent living skills of program participants to promote their personal well-being, self-sufficiency and integration in the community.
  • Integrate Supportive Housing services with other community-based rehabilitative services provided by Laurel House Resource Center staff such as employment, education, wellness and psychosocial rehabilitation.


Provide individualized support and services to assist program participants in developing strengths, skills, and contacts with community resources.  Demonstrate empathy, respect and consideration for the dignity and self-esteem of all participants, while providing skills-training and support to promote independence and optimize quality of life.

  • Provide case management to 15-20 individuals with serious mental illness and some of those with co-occurring disorders such as substance use disorder
  • Develop an individualized Recovery Plan with each tenant, directed at restoring participant functioning and achieving independent participation in community activities and, longer term, full integration into their local communities.
  • Provide independent living skills counseling, coaching, and assistance to participants in Laurel House’s Community Support and Housing programs in the following areas:
    • self-care and proactive scheduling and follow-through for medical appointments;
    • maintenance of apartment or other living environment;
    • self-advocacy and development of socialization skills (goal setting, problem solving, communication, etc.).
    • tenant interrelations and rights and responsibilities of community living;
    • benefits and related reporting requirements;
    • money management, budgeting, bill paying, banking;
    • engagement in the local community and in skill building programs.
  • Create meaningful recovery plans; update and maintain records for tenants, including vital statistics with current contact information, service plans, reassessments, progress notes, and all other records and reports required by program funding sources, in both paper and electronic versions.
  • Provide outreach, support, and continuity of service in apartments, housing office, and community.
  • Facilitate program participants’ use of Laurel House Social Rehabilitation (e.g., skill building workshops, Thinking Well - Cognitive Remediation); food service; social events; Supported Education and Supported Employment.
